SWAFO implements discipline policies outlined in the Student Handbook to ensure a safe, respectful, and fair campus environment. 

This includes handling: 

• Minor and major student violations 

• Code of Conduct concerns 

• Discipline-related guidance and clarification 

A structured program for students who committed major offenses and were given suspension or alternative sanctions. 
 

The program focuses on: 

• Accountability and reflection 

• Learning from mistakes 

• Personal growth and character formation 

Who needs to undergo this program? 

• Students who committed a major offense (enrolled, transferred, or graduated) 

• Students who have already completed their sanction 

A preventive education program that: 

• Promotes discipline awareness 

• Encourages respect and responsibility 

• Helps reduce minor and major violations 

This program introduces students to discipline expectations and the role of SWAFO in campus life. 

SPADE Program Objectives 

• Introduce the role, programs, and services of SWAFO in the University 

• Educate students on the importance of discipline inside the campus 

• Reduce the incidence of minor and major offenses 

• Answer the frequently asked questions (FAQs) about offenses and corresponding sanctions 

The Vanguards of Student Discipline is a student-based service program that supports campus order and discipline. 

VSD members: 

• Serve as role models of proper conduct 

• Help observe and report campus concerns 

• Promote a culture of responsibility and respect 

Who may apply? 

• Volunteer students 

• Formation Program completers 

• Students under probationary status 

For students with traffic-related violations, SWAFO assists in processing and resolution. 

Please bring: 

• Traffic Violation Ticket 

• DLSU-D ID 

• Original printed Certificate of Registration (COR) 

Transactions are processed in person at the SWAFO office. 

SWAFO processes requests for Certificates of Good Moral Character, subject to clearance and evaluation based on student records and compliance. 

ONLINE APPLICATION FOR CERTIFICATE OF GOOD MORAL CHARACTER 

1. Send email to swafo_helpdesk@dlsud.edu.ph to apply, then wait for the Application Form link. 

2. Please read the instructions carefully then fill out the Online Application Form. 

3. Confirm application thru email the request for payment slip and instructions 

4. After online payment, send proof of payment with complete payment details 

5. Wait for acknowledgement email and Print Transaction Slip/Claim Stub 

Reminder: Make sure to READ and FOLLOW the stated instructions to avoid delays processing your request. 

SWAFO handles transactions related to agreements for membership in fraternities, sororities, and unrecognized organizations.

“Lasallians Are Honest” Campaign 

SWAFO serves as the official custodian of all surrendered lost and found items on campus. 

Services include: 

• Receiving and safekeeping of lost items 

• Coordination with proper offices 

• Claiming and endorsement procedures 

Visit the SWAFO office at the GMH Building 

Students who return valuable items may receive: 

• Certificate of Honesty 

• Nomination for Luntiang Parangal
